The medal's obverse features a design after Lloyd Ostendorf's original artwork that depicts Lincoln serving as a store clerk. On the reverse, a short biographical sketch regarding his move to Illinois appears. A laurel wreath borders the lower half of the medal. The medal is mounted in a leather slipcase album, which is Volume II of a two-volume set.
