The medal's obverse design depicts a bust image of Ulysses S. Grant. Grant is shown wearing his Union uniform. Behind the bust, a scene from the Vicksburg Campaign is depicted. Two riders on horseback are shown along with a cannon and approximately four soldiers holding either rifles or flags. A lone bugler is also shown. On the reverse, Confederate and United States flags are pictured intersecting at the top of the medal. Several rifles appear behind the flags, and the image of an eagle is placed at the center of the intersection. At the medal's bottom, a decorative border lines the edge.
