The medal's obverse design depicts Robert E. Lee and Ulysses S. Grant seated at a table. Lee is featured on the left side of the design, while Grant sits at the right with his hat on his lap. On the reverse, Confederate and United States flags are pictured intersecting at the top of the medal. Several rifles appear behind the flags, and the image of an eagle is placed at the center of the intersection. At the medal's bottom, a decorative border lines the edge.
